We are so grateful for the blessings of the Lord here at Rainbow Praise Church and are looking forward to even greater things in the weeks and months ahead. Rainbow Praise Church has been given the opportunity to host a COVID-19 testing session on Friday, May 22, 2020. The testing will be conducted by Wellness Group/PMH Laboratory Inc. for both COVID-19 (QPCR test) and COVID-19 Antibody testing.
Testing is available for anyone and is free with proof of insurance. No co-pay is required - testing is billed directly to your health insurance provider.

· When making an appointment: Under "Select Service", choose "COVID-19 Test" (both COVID-19 Diagnostic and COVID-19 Antibody tests are available). The calendar will display May 22. Click on May 22 to view a list of
available times and select a time slot.
· Note: You must register individually, you cannot register your family under one name. If you wish to register a dependent/family member, log out and log back into the online appointment scheduler.
· Required: Complete and bring the following form to your appointment: COVID-19 Test Request Form
· Required: Review the PMH Laboratory Privacy Statement
· Required: Bring a copy of your health insurance card (both front and back) to your appointment
· Required: Bring a copy of your driver's license or valid government issued ID to your appointment
· On the day of testing, wear a shirt with short sleeves and drink plenty of water. Wearing of a Mask is REQUIRED during the testing.
· For persons with no medical insurance (uninsured), the cash cost for the COVID-19 Antibody Test is $125 and COVID-19 QPCR Test is $175. Here is a link for UMC COVID-19 diagnostic testing sites that offer low to no cost
